Supply the actual lesson
A study coach should receive the relevant lesson text and code, then cite the passage or line behind its explanation. If the required material is unavailable, the system should say so. A confident answer is not evidence that the source supports it.
Version the context
Store a content version or hash with generated questions. When the lesson changes, mark dependent questions for review instead of silently treating old answers as current. Refresh deliberately and keep an audit of what generated the content.
